Before you download the registration form at the bottom of this page, please take the time to read this important information on FOCUS policies.
Please note the following policies:
Payment: The full program fee or notice of scholarship is due at the time of registration. You may submit a Visa or MasterCard number, or enclose a check with your registration forms.
Internships: Applicants for Internships, Service Programs, and Volunteer Leaders must have approval from the Program Director before they register.
Cancellation: Our cancellation and refund policy is as follows: full refund with more than 30 days notice; 50% refund with 15-30 days notice; no refund within 15 days, except in case of emergency.
Registration Process:
Download: Download the registration form below, fill it out, and mail, email, or fax it to the FOCUS Program Office. You may also submit the registration form that is found on the brochure.
Forms: To complete registration, you need only submit the registration form. However, before attending a program you must submit additional required waivers.
Confirmation: Once we have processed your registration, we will send you a confirmation email within three days. If you have not heard from the Program Office within 10 days of mailing your registration, please contact programs@infocus.org or (434) 245-6112.
